Membership Form
1. To apply for membership to CIC fill in the details after printing the form and post it to us with the payment and a photocopy of your Driver’s license or passport. We will acknowledge your mail and post the receipt for the membership fee. Your membership commences on the day when the CIC executives approve your application. Direct Deduction from your bank account
2. Print the form, fill in the details and post or present it to your bank. Some banks do charge a fee for this service. We will post you a receipt regularly [fortnightly, monthly depending upon the frequency of the deposit] provided you post a copy of the above form to us also. CIC keeps such details confidential. Some banks give sufficient details in their statements so that we can tally the donation with the donor’s details. Some banks do not give this information to us. Should your bank keeps this information about your account confidential then you can ask us for a comprehensive statement detailing all your deposits during a fiscal year when you do your tax returns. Direct Deduction from your salary
3 Print the form and fill in the details and then pass it on to your payroll office. Payroll office does not charge you for this service and all direct deductions to CIC will appear in your pay-slip. Click here to access the direct deduction from your salary form.
Hiring conditions and
application form for hire
4. The facilities at
the CIC can be hired for family functions subject to the conditions that
regulate hiring. The conditions are printed on the first page and the
application form is on the second page. Please take time to read the conditions
before filling out the form. The current rates for hire are given with the
conditions. Centre can also hire out AV equipment such as audio tape, Video and
DVD players, sound system, Overhead projectors, Data projector, Electronic and
white boards and projection screen, disposable crockery and cutlery and table
covers etc. The chairs and tables [enough for 35o guest are included in the
hiring of hall. Kitchen outside the hire of halls is $25 per hor. A cleaning
bond of $100 is required with all hiring, returnable when premises are left in a
